Overview.

Our client, one of the leading US-based healthcare providers, was struggling with two features (patient scheduling and referral management) in its EHR platform. Both procedures, though vital for revenue cycle management, are often common pain points for healthcare providers. The client was facing issues with its EHR (Electronic Health Record) platform which allowed patients to access healthcare services and information in one place. The client wanted to revamp its EHR platform to improve customer experience and streamline the patient scheduling process. Coforge modernized the core application and delivered the exact solution the client was looking for.   

The Challenge

  • Adding New Features  

The client required a solution regarding patient experience for the EHR platform. Before the revamp of the platform, patients used to schedule and pre-register appointments separately, causing inconvenience. The client wanted both the features in a “single window”.   

  • Operational Hurdles  

The implementation of the solution had operational challenges as the processing of sending an Order Packet to the healthcare provider’s financial clearance center for scheduling and registration was broken.   

  • New UI  

  To support the “single window” operation, a new UI was required.

Coforge, as the app development partner of the client, set up 2 teams to individually focus on improving and stabilizing the core foundation of the platform.   

Our structured approach involved the following steps:  

  • Refactored the existing monolithic components into deployable services to enable the “single window” application development  
  • Developed application functionalities across various services, like: 
  • Patient Demographic Capture 
  • Eligibility and Benefits Determination  
  • Cost Estimation  
  • Coverage Exceptions
  • Authorization Exceptions 
  • Medical Necessity 
  • Financial Counselling 

Adopted microservices and domain-driven design principles  

The Coforge team revamped the following areas of the core application:  

  • Built a DevOps pipeline for this development  
  • Consolidated the middleware platform to SoftwareAG 
  • Built new Framework components on .NET Core V3+ 
  • Migrated to Linux servers for the application tier  
  • Introduced Elastic infra, e.g. Kubernetes management, on the client’s existing IBM/Skytap Cloud platform  
  • Migrated to NoSQL from existing MS SQL Server database 
  • Application performance improvement 
  • Drove the migration of applications from the current IMB/Skytap environment to AWS and adopted serverless architecture

The Impact

We achieved the following outcomes:  

  • Drove synergies and offered cost and productivity improvement benefits to the customer by leveraging our multiple capabilities, such as Application Engineering and Legacy Modernization with microservices design, integration services redesign, DevOps, QA Automation, Cloud adoption (IBM to AWS), and Infra (app servers and databases) migration.  
  • Introduced outcome-based development with the introduction of SLAs for releases and sprints, which resulted in better cost and operational efficiency for the engineering team.
